General Appointment

Please note that the instructions below are for appointments for non-weight loss consultations. If you are seeing us for breast, colon, thyroid, abdominal, hernia, and other general surgical consults, these instructions are for you. If you are interested in seeking a consultation for weight loss surgery, you must follow the Bariatric Program Quick Link to the right, for the specific instructions and forms.

Please download and complete our New Patient Forms prior to your first office visit. Please note your previous surgeries and any medications you are taking.

We are open Monday to Friday (except Holidays) between 8:00am to 4:30pm.

As a Specialist clinic, in most cases patients need referral from their General Practitioner, family physician or other doctor in order to be covered by insurance.

We are located on the second floor of the Shelby Medical Building, the office to the right of Shelby Baptist Medical Center, across from the new bed tower and parking deck..

When you come for your appointment, remember to bring the following:

  • Driver’s License
  • Insurance Card(s)
  • Copies of operation records, medical records, x-rays, MRIs, CT scans and so on from prior doctor visits.


To expedite your visit, you can complete some of your New Patient paperwork here. Please print completed forms and bring with you.

To cancel an appointment:

Telephone the office during business hours. Please cancel at least 1 day ahead so that your appointment time can be allocated to another patient who is seeking treatment.
